Due to expansion within the Stockport Home Treatment Team an exciting opportunity for 2 full time Early Discharge Coordinators
Your main base will be at Stepping Hill Hospital.
This post is open to Nurses, Occupational Therapists or Social Workers.
The Stockport Home Treatment Team is a large multi-disciplinary team, consisting of a Consultant Psychiatrist and junior doctors, Team Manager, Deputy Manager, Psychologist, NMPs, Lead Nurses, Occupational Therapist, Social Workers, Support Workers, Pharmacy Technician and Administration staff.
We are in an exciting innovative position of piloting this new role within the team, which means enhancing our support offer on the acute wards, supporting early discharge to the Home Treatment Team.
The postholder will work closely with both the Home Treatment Team and the acute wards at Stockport, enabling a smooth discharge from hospital.
This well-established, multi-disciplinary team offers a high quality assessment and home treatment service where possible as an alternative to hospital admission, to individuals experiencing acute mental health crisis.
We are currently working to enhance our team in line with the Fidelity model for Crisis services in conjunction with all HTTs .
You will work within the service specifically as part of the MDT and have close links with the consultants on the wards and the Psychiatrist and Psychologist from the Home Treatment Team.
You will participate in Ward rounds for patients on the ward, coordinating patient discharge. Additionally, you will work with other colleagues across assessment services to enhance care provision.
We are looking for an individual who shows compassion and genuine warmth, someone who is engaging and focussed on person centred care and satisfaction.
You will be able to keep our commitments, demonstrate respect and dignity, work well under pressure and have very good communication skills, you will be working in a fast paced workplace where diversity is a focus.
You will have experience as a qualified practitioner within mental health and have strong leadership skills.
