An exciting opportunity has arisen for a Nurse to join our Community Rehabilitation Team (CRT) based in Queen Margaret Hospital, Dunfermline. CRT has the overall aim of improving the quality of care and outcomes for older people with the intention of allowing them to remain independent within their own homes or communities. A key aspect of the role is supporting the patient flow through the health system by facilitating early supported discharges from the acute hospitals and also by preventing admission to hospital. This will be done by providing nursing interventions to patients on the caseload, and also by taking on a key worker and generic assessor role within the team. This post is essential part of the multidisciplinary team which also included physiotherapists, occupational therapists and healthcare support workers.
You should have excellent communication skills, be able to demonstrate sound clinical skills required for the role, decision / reasoning skills, and good time management/organisational skills. You should be able demonstrate person centered care, have good understanding of rehabilitation and multidisciplinary working and have a good understanding of the local community and resources, with community experience preferred.
You will require to have NHS experience of working with adults, mainly older people and have assessment and treatment knowledge and skills associated with this patient group.
You will require to have a diploma in Nursing and be registered with the NMC. Evidence of continuous professional development appropriate for the banding is essential.
There may be occasions where you will be required to work in one of the other community rehabilitation team sites.
Our service offers regular supervision, PDP and supports training opportunities.
The ability to travel throughout the area is essential as this post is community based.
This is a shift work position that includes evenings and weekends.
Public Holiday working is in operation (accept Christmas Day and New Year’s Day).
