As a Learning Disability Well-Being Practitioner and a member of Well -Being Practitioners Team you will play a vital role in ensuring patients with learning disabilities receive safe compassionate and person- centred care during their hospital stay. You will also support individuals with a to access acute healthcare services by delivering practical and emotional support, including one-to-one assistance during hospital appointments and admissions, promote and facilitate the use of reasonable adjustments and accessible communication tools (e.g., Easy Read materials, Hospital Passports) to reduce health inequalities and improve patient experience under the supervision of the Learning Disability Nurse.
This role requires empathy, adaptability and teamwork. You'll need strong communication skills, the ability to stay calm under pressure, and a commitment to improving patient experience.
The post holder will be expected to work with patients admitted to the Emergency Department and Inpatient wards. As a member of the multidisciplinary team, delivering a high standard of evidence-based care and treatment in order to maximise each patient individual potential.
Deliver person-centred care to patients with learning disabilities and autistic people during their hospital stay.
Support registered nurses and the multidisciplinary team with clinical and non-clinical tasks.
Engage patients in activities and provide emotional support to them and their families.
Complete documentation such as Learning Disability and Autism hospital passports.
Assist with audits, surveys, and staff training across the Trust.
Communicate effectively with patients, carers, and staff, adapting your approach to individual needs.
